We want to ensure the very best quality of care is available to our patients and communities, and that it is sustainable into the future from buildings which are fit for purpose.
At the heart of our vision is wanting to keep you well, and for as much care to be delivered as close to your home as possible.
We want to do this in a joined-up way with GPs and clinicians from hospitals, community and mental health organisations, working together alongside social care practitioners and the voluntary sector.
We also need to ensure that when you are seriously unwell or at risk of becoming seriously unwell, you have access locally to the highest quality care, available at any time of day or night and on any day of the week.
